Position Title: Bilingual Licensed Clinical Social Worker
Reports to: Executive Director Full Time
The Licensed Clinical Social Worker provides counseling to CCC patients as well as works within an integrated model of care with the clinical staff to improve the overall wellness of patients. The LCSW or LCSWA will meet weekly with the clinical staff to discuss the overall health of individual patients to improve health outcomes. Responsibilities specific to Case Management include assessment, identification of specific needs, and social service intervention.
Principle Activities:
•Provide an array of mental health services, including but not limited to screening, assessment,diagnoses, evaluation, crisis intervention, consultations, care coordination/case management,education, referral, and other related services
•Conduct eligibility and social determinants of health screenings
•Conduct intakes, individual, group, and family therapy
•Develop service plans and conduct reviews as needed throughout the duration of treatment
•Complete and maintain documentation in a timely and accurate manner to Federal, State andCCC guidelines in EPIC
•Write reports to court, school, or other community agencies as necessary
•Follow up with psychiatrists and/or attending physicians in cases involving hospital admissions
•Produce and disseminate reports on the total of patients seen
•Collaborate with external psychiatric care providers as well as representatives from civicassistance programs to help schedule treatment sessions for those patients who need them
•Other related activities designed to provide integrated healthcare services
Knowledge, Skills, Abilities and Personal Characteristics
•Must be bilingual in English and Spanish
•Have a current practitioner license through the state board
•Must be accredited or working towards accreditation if currently an associate
•If operating as an associate, must be under the supervision of a fully licensed clinical socialworker with a sufficient amount of post-graduate counseling experience.
•Excellent verbal, interpersonal, and relationship-building skills
•Proficiency in EPIC is a plus, as well as strong computer literacy skills
•High energy, positive, “can-do” attitude, flexibility, teamwork, and attention to detail; highdegree of initiative and ability to work independently.
•Able to make professional evaluations, decisions, and recommendations for treatment planningand implementation
•Ability to build rapport with patients
•Familiarity with community mental health resources
•Knowledge of chronic disease issues
•Serve as part of the medical home healthcare team
